Disparity Outreach
A Health Care disparity is a lack of medical services in certain populations in the United States. A healthcare disparity involves those groups affected due to their race, religion, and/or their socioeconomic status. Lack of insurance, financial reasons, a cultural belief and language barriers can impede access to health care. Patient Advocate Foundation provides direct case management assistance and outreach to those populations.

PAF provides assistance to African American communities through case management services and outreach. The Patient Advocate Foundation African American Outreach Program and its case managers are available to provide assistance to members of African American communities who have high risk medical conditions and are finding it difficult to access needed medical services.

The Patient Advocate Foundation provides direct case management assistance and Outreach to the Hispanic/Latino Communities. The Patient Advocate Foundation National Hispanic/Latino Outreach Program and its bi-lingual case managers are available to provide assistance to members of Hispanic/Latino communities in the United States who are having problems accessing health care.

The Patient Advocate Foundation understands our aged and disabled population is extremely fragile. The Patient Advocate Foundation Senior Services Department provides direct case management services and outreach to the Senior and disabled populations who need assistance understanding Medicare benefits, such as Advantage Plans and Medicare Part D enrollment assistance; and all issues related to accessing to healthcare.
